Occurrence Reporting
A guide to fill the form GST-10 and 11
Serious incidents and accidents shall be reported immediately to AAIB, tel. 5111666 or 112. The Occurrence shall also be reported to ICAA on form available on the ICAA website. The form is available both in MS Word and PDF format. You can either fill out the Word document in a computer or print it out and fill it in with a pen. Choose form GST 10 for an English version. Then send it as an attachment to mandatory.reporting@caa.is. If you print out the form please fill it out and fax it to 588-4288 or send it by mail to Icelandic Civil Aviation Administration, Skogarhlid 12, 105 Reykjavik.
The areas that are available for you to fill out are:
- 1. TYPE OF OCCURRENCE: Check the appropriate box that best describes the type of
occurrence you had.
- 2. Place into AIRCRAFT INFORMATION the information regarding registration, date of occurrence SQUAWK, where the occurrence took place etc. SQAWK is important e.g. when loss of separation occurs or when other type of ATC or vortex occurrences take place.
- 3. In the PURPOSE OF FLIGHT section, check the appropriate box. You can check more than one box here, since for example a charter flight can also be a passenger flight or a freight flight.
- 4. In the FLIGHT PHASE section you need to check in what part of the flight the occurrence took place. Below you write number of passengers and crew, what flight rules you were following, airspeed in Knots indicated, take-off mass and ATA code for technical failures (if available).
- 5. In the CONSEQUENSES section you need to check and writhe if necessary the
consequences of the occurrence.
6. In the CONFIGURATION AT EVENT section write in the configuration of the aircraft
when the occurrence took place. Skip this part if the incident occurred on ground.
- 7. In ENVIRONMENTAL DETAILS section, you need to give information regarding weather, runway, light conditions and wind shear. Skip this section in case of a GROUND FOUND.
- 8. BIRD STRIKE box needs to be filled incase of a bird strike or turbine ingesting a bird. If possible fill out the bird type name in English.
- 9. In the GROUND FOUND box write a description of damage found during pre- or postflight inspection.
- 10. WAKE TURBULENCE box needs to be filled if the aircraft experienced a wake turbulence during the flight.
- 11. Into SHORT GENERAL DESCRIPTION OF OCCURRENCE you need to write with your own words a description of events in chronological order. If the nature of the occurrence does not fit into any particular box please write the description into this field. Use short sentences that describe the facts in chronological order.
- 12. ACAS/TCAS warnings shall be written into the AIRPROX / ATC PROCEDURAL and/or TCAS RA box.
